     Treatment of Hypophosphatemia:

Recent and uncomplicated hypophosphatemia

Phosphates solution 500mg (16mMol, 4mL) 2-4 times daily

Symptomatic hypophosphatemia

15-30 mMol PO43- IV given in 1-2 doses

Higher doses up to 0.5 mMol PO43-/kg have been used for serum
PO43- < 0.16mMol/L. Maximum dose 45 mMol/24 hrs.

(sodium phosphate formulation preferred with higher doses due to no potassium content).  

Additional doses should be guided by serum phosphate levels taken 6-24 hours after the infusion. Also, check serum potassium and calcium levels.

Potassium phosphate

 

Maximum peripheral concentration is 15 mMol PO43- (5mL) in 250 mL NS or D5W (higher concentrations must be run centrally).  Infuse over
4-6 hrs.
 

Sodium phosphate

 

Dilute dose in minimum 100mL NS or D5W.  Infuse up to 15 mMol PO43- over 2 hrs; infuse > 15 mMol PO43- over 4-6 hrs.
