7.2 ADVERSE DRUG REACTION REPORTING
POLICY
All suspected adverse drug reactions (ADRs) reported
to the pharmacy department or identified by a pharmacist will be
investigated by a clinical pharmacist. All unexpected or significant
ADRs will be forwarded to the B.C. Drug and Poison Information Centre (DPIC).
PROCEDURE
- Yellow ADR alert forms are available on every
nursing unit.
- When a suspected ADR is reported to the
pharmacy department or identified by a pharmacist, an ADR report
form will be completed by a clinical pharmacist
- Completed ADR report forms will be FAXED to the
B.C. Regional ADR Centre upon completion (806-8262). Original to be
filed in the ADR file.
- Once a year, a summary report is prepared and
published in the Drug & Therapeutics Newsletter and reported to
the Drugs and Therapeutics Committee
