INCIDENT REPORTING POLICIES:

7.1 REPORTING MEDICATION ERRORS

POLICY

All medication errors are to be reported immediately, charted and appropriate follow-up implemented.

PROCEDURE

Person discovering error:

  1. Assesses patient for injury immediately and initiate appropriate corrective action. If medical intervention required, notify physician.
  2. Notifies manager/supervisor of the area.
  3. Completes Incident Report form.

Manager/Supervisor:

  1. Ensures appropriate action has been taken.
  2. Investigates error and documents follow-up action taken.
  3. Implements corrective action to prevent error from recurring.
