MEDICATION ADMINISTRATION POLICIES:

 

5.8 GENERAL GUIDELINES FOR PATIENTS UNABLE TO SWALLOW WHOLE TABLETS

 

1.   Most regular release tablets may be crushed and either diluted in water or sprinkled onto soft food such as applesauce.

 

2.   Most regular release hard gelatin capsules may be opened and the contents sprinkled onto applesauce or similar soft food.

 

3.   Long-acting formulations of tablets or capsules should not be crushed or chewed. These are tablets or capsules may have
designations such as:

 

      SR (sustained release)

XR (extended release)

ER (extended release)

CD (controlled delivery)

LA (long acting)

XL (extra long)

CR (controlled release)

MR (modified release)

 

4.   Enteric coated medications should not be crushed unless a prescriberís order is obtained.

 

5.   Cytotoxic medications should not be crushed. Contact pharmacy to discuss alternative formulations.

 

6.   Some medications may be available in a liquid, buccal, wafer, or other alternative form. Nursing to call pharmacy to
switch to an appropriate liquid or alternative formulation.

 

7.   Questions regarding how to administer a medication or if a review of patientís medications is required, the unit
clinical pharmacist should be contacted. During the evenings and weekends, a pharmacist may be contacted in the dispensary.
