5.0 MEDICATION ADMINISTRATION POLICIES

 

5.4 GUIDELINES TO STANDARDIZE PARENTERAL MEDICATION DOSING TIMES

Purpose:
To assist nurses to stagger the next parenteral medication administration time to return to standard dosing times.

Note that this is just a tool; always use nursing judgement when adjusting parenteral therapy times.

Standard dosing times:
Q4H 0200, 0600, 1000, 1400, 1800, 2200
Q6H 0600, 1200, 1800, 2400
Q8H 0600, 1400, 2200
Q12H 1000, 2200

Heparin 5000 units subcutaneous ordered as TID or Q8H = 08-16-22
Vancomycin ordered as Q12H = 09-21

Exclusions:

Chemotherapeutic Agents, Diuretics, Immunosuppressives (CYCLOsporine, mycophenolate, tacrolimus)
