Definitions
“CLEAR WINDOW” is time interval when a dose can be administered
“COLOURED WINDOW” is time interval to wait until the next dosing time to give a dose
“HIGHLIGHTED HOUR” is the standard dosing time for scheduled drugs
Medication Dosing Times
There is an allowance of +/- 60 minutes around the scheduled time.
Oral medication orders written as Q12H, Q8H, Q6H will be changed to bid, tid, and qid except those on the exception’s list
PROCEDURE
Nurse: 1. Find appropriate dosing interval.
2. Locate window with the time of medication order and/or availability
3. Follow window instructions for first dose.
4. Give all other doses following at the highlighted times.
5. May take up to 24hrs to adjust to standard dosing times if clinical situation dictates.
Exceptions: Warfarin – give at 1700 hr
Once daily oral antibiotics ordered between 2000 & 0700 (give 1st dose now)
