5.0 MEDICATION ADMINISTRATION POLICIES:

5.2 STANDARD ORAL MEDICATION ADMINISTRATION TIMES: VANCOUVER ACUTE 

POLICY:

 1. All orders for oral medications will be processed with the standard times except for the following:

a)     medications ordered with specific times of administration

b)     medications listed on the Standard Oral Medication Frequency – Exception list

2.  Orders for oral medications that are written as Q12HR, Q8HR, Q6HR will be changed to bid, tid, qid

3.  The window of time for medications to be administered is 60 minutes before and after time stated on the MAR

4.  All new orders for an increase in dose should be acted upon at the next scheduled dosing time unless otherwise indicated by a prescriber’s written or verbal order.
